James F. McGuckin, Jr., M.D.
Chief Executive Officer
Founder, VAC 2005.

Dr. McGuckin is Director of the Philadelphia Vascular Institute located in Philadelphia, Pennsylvania. Dr. McGuckin’s research interest include: Cardiovascular Systems, Endoscopy and Endosurgery, Peripheral Arterial Disease, Limb Salvage, Percutaneous Oncologic Therapy and therapy related to the End Stage Renal Disease population. He is currently researching recanalization of Central Venous Occlusion using RF ablation.

He holds 39 patents and has over 170 pending patents. Dr. McGuckin double majored in Mechanical Engineering and Pre-Medical from the University of Notre Dame in 1983, and received his M.D. from Hahnemann University in 1987, General Surgery in 1988, Masters Degree in Bioengineering at the University of Pennsylvania in 1990, Diagnostic Imaging in 1995, and completed his Fellowship in Interventional Radiology at the Hospital of the University of Pennsylvania in 1996.

PENNSYLVANIA

Sandeep Sharma, M.D.
VAC of Pittsburgh

Dr. Sandeep Sharma was one of the first doctors to bring Interventional Nephrology to Pittsburgh. He has been in practice since 2006 and routinely services patients not only from Pittsburgh, but also from across Western PA to Ohio and West Virginia. Dr. Sharma has received Board Certifications in Family Medicine, Internal Medicine, and Nephrology. He is also certified by the American Society of Interventional Nephrology. Dr. Sharma received his training in interventional procedures from renowned interventionalist, Dr. James McGuckin, and Vascular Surgeon, Dr. John Ross.

He has given presentations at National conferences as well as locally for the Fistual First initiative. He routinely provides education to dialysis nurses and technicians on the intricacies of access management. He is a board member of the National Kidney Foundation and a strong proponent of patient education. He is active within the community to educate people about kidney disease and transplantation. Dr. Sharma specializes in all aspects of hemodilalysis access maintenance, arm and chest port placement, and varicose and spider vein treatment.

David Catalane, M.D.
VAC of Pittsburgh

David Catalane, M.D., is a board certified General/Vascular Surgeon with over 25 years experience treating vascular disease. A graduate of the University of Pittsburgh School Of Medicine with honors in surgery, he did his surgical training in Pittsburgh at Mercy Hospital. Dr. Catalane is on staff at St Clair, OVGH and Mercy to care for in-patient hospital needs but is now able to offer outpatient vascular evaluation and treatment at the Vascular Access Center of Pittsburgh in the new Cranberry location.

Dr. Catalane has successfully endeavored to keep abreast of all the latest advances in surgical care, including minimally invasive endovascular techniques. These procedures make possible the management of complex vascular problems, many requiring only a needle stick or single, small incision. Problems treated include: peripheral vascular disease, non-healing wounds, threat of limb loss, dialysis access placement, aortic aneurysm, venous disease and others.

Paul Willis, M.D.
VAC of Pittsburgh

Dr. Paul Willis is a Pittsburgh native, and graduate with honors, of the University of Pittsburgh’s School of Arts and Sciences and School of Medicine. He completed his surgical training and a research fellowship at Bassett Healthcare, Cooperstown, NY, where he also met his wife. After completion of his training, he moved back home to Pittsburgh, where he and his wife started a family, and he has been actively practicing General and Vascular Surgery ever since (16 years).

As a partner of Catalane and Willis Surgical Associates, Dr. Willis has also kept abreast of all the latest advances in surgical care, including minimally invasive endovascular techniques. These procedures are minimally invasive with little to no downtime required. Problems treated include: peripheral vascular disease, non-healing wounds, threat of limb loss, dialysis access placement, aortic aneurysm, venous disease and others.

Mike Watts, M.D.
VAC of Main Line

Dr. Mike Watts is an interventional radiologist dedicated to providing the highest level of service through clinical excellence and a commitment to patient and family centered care.

Dr. Watts graduated from Colgate University in 2002 with a Bachelor of Science in Chemistry. He then attended the University of Buffalo for his medical training, earning his MD in 2006, followed by completion of his surgical internship and radiology residency at the Cleveland Clinic in 2011 and his fellowship at the University of Pennsylvania in 2012.

Dr. Watts specializes in advanced peripheral arterial disease techniques, minimally invasive cancer treatments, venous disease, and management of portal hypertension. His expertise in these therapies has made him a well renowned invited speaker at various regional, national, and international conferences. Committed to education, Dr. Watts acted as the Interventional Radiology fellowship director at the University of Pennsylvania and created one of the first Interventional Radiology residencies in the country. His devotion to hands-on teaching of complex techniques has driven him to continuous improvement through a commitment to life-long learning and innovation of novel procedural techniques.

NEW JERSEY

David Soto, M.D.
VAC of West Orange

Dr. David Soto is a Board Certified Diagnostic and Vascular Interventional Radiologist. He completed his subspecialty fellowship training at Stanford Universit Medical Center in 2010 and has practiced as an Interventionalist in a variety of settings in northern California for the last 5 years. During that time, he worked for a large hospital system, spending all of his time performing a broad range of vascular and nonvascular procedures, and also in a standalone interventional practice - in which he provided inpatient and outpatient vascular and oncologic services. Before coming to VAC, Dr. Soto was in a busy community hospital practice setting with emphasis on uterine fibroid embolization, veno-occlusive disease, peripheral arterial disease, kyphoplasty/pain management, and dialysis access maintenance.

Dr. Soto obtained his Bachelor of Science degree in Biomedical Engineering at the University of Pennsylvania and his medical degree from the State University of New York at Downstate Medical Center. He went on to complete an Internal Medicine preliminary internship at Staten Island University Hospital and a year of Nuclear Medicine Residency training at Albert Einstein College of Medicine before completing the Diagnostic Radiology Residency program at Temple University Medical Center. Dr. Soto obtained additional focused, rigorous training in endovascular peripheral arterial disease therapies at the Peripheral Vascular Institute of Philadelphia under Dr. James McGuckin before joining the team at the Vascular Access Center of West Orange. As a New York City native, he is excited to return to the East Coast and to have the privilege of providing endovascular and interventional excellence to the surrounding community.

Scott Hollander, M.D.
VAC of Atlantic County

Dr. Hollander is a Diplomat of the American Board of Radiology with Certificate of Added Qualification in Vascular and Interventional Radiology. Dr. Hollander obtained his Doctor of Osteopathy at the New York Institute of Technology, followed by his Diagnostic Radiology residency at Columbus University Medical Center. Dr. Hollander then went on to complete his fellowship in Vascular Interventional Radiology at Yale-New Haven Hospital in Connecticut. He remained in Connecticut for several years as the Director of Interventional Radiology at Stamford Hospital before taking an Interventional Radiology position spanning across the tri-state area.

Dr. Hollander recently joined Philadelphia Vascular Institute where he obtained additional Peripheral Arterial Disease training from James, McGuckin, M.D. at the Peripheral Vascular Institute of Philadelphia.

Dr. Hollander is an avid sports enthusiast and historian with a passion for health and fitness.

Stephen Kolakowski, M.D.
VAC of Eatontown

Dr. Kolakowski is a board certified vascular and general surgeon who attended Ross University School of Medicine. He later trained in General Surgery at the University of Pennsylvania where his rotation included general surgery, vascular surgery, cardiothoracic surgery, trauma, Pediatrics, Burns, Urology, Neurosurgery, ENT, Orthopedics and Sarcoma surgery. He completed his vascular surgery fellowship at Pennsylvania Hospital.

Dr. Kolakowski is a fellow with the American College of Surgeons as well as member of the Society for Clinical Vascular Surgery.

Dr. Kolakowski has published numerous articles and chapters in the field of Vascular and Endovascular surgery. He specializes in both endovascular and open surgical repair of peripheral arterial disease, limb salvage, carotid artery disease, abdominal aortic aneurysm repair, dialysis access and varicose vein treatment.

Mir Ahmad, M.D.
VAC of Central New Jersey

Dr. Mir Ahmad, M.D. completed his Internship and Residency in 1998 from Mount Sinai School of Medicine-VA Bronx program and then his Fellowship in Nephrology in 2000 from UMDNJ-Cooper Medical Center, Camden, NJ.. Dr. Ahmad then trained in Hemodialysis Vascular Access under Dr. James McGuckin, M.D. and is certified by the American Society of Diagnostic and Interventional Nephrology.

Dr Ahmad has been practicing as a Clinical and Interventional Nephrologist in New Jersey since 2000. In addition he is a Medical Director of FMC – Maplewood, NJ since 2005. He is Board Certified in Internal Medicine and Nephrology by the American Board of Internal Medicine and holds active Medical Licenses in New Jersey. He is also trained in Endovascular management of Peripheral Arterial Disease. Dr. Ahmad specializes in all aspects of hemodialysis vascular access, fistula maturation, chest port placement, peripheral arterial disease and vascular medicine.

William D. Yu, M.D., F.A.S.N.
VAC Prince George’s County & Washington, DC

Dr. William Yu graduated from Boston University School of Medicine in 1994 as part of the 6-year combined B.A./M.D. program, from which he graduated Magna Cum Laude. He completed his Internal Medicine Residency from 1994-1997 and his Nephrology Fellowship from 1997-1999, both at Northwester University School of Medicine in Chicago. He is Board Certified in both Internal Medicine and Nephrology. He has practiced in a private practice setting for over 6 years in the Baltimore area. Despite being in private practice, Dr. Yu was active in teaching and had a part-time academic position at Johns Hopkins School of Medicine as a Clinical Instructor of Medicine from 2000-2005. Furthermore, Dr. Yu was awarded the “Faculty Teacher Award” in 2001 by the Johns Hopkins University School of Medicine and Sinai Hospital Program in Internal Medicine.

In 2001, Dr. Yu was trained in Interventional Nephrology procedures, specifically Hemodialysis Vascular Access procedures, by Interventional Radiologist Dr. W. Perry Arnold as well as Interventional Nephrologist, Dr. Gerald Beathard. By the beginning of 2002, Dr. Yu was the first practicing Interventional Nephrologists in the Maryland and Washington, DC area. In early 2004, Dr. Yu was one of the first Interventional Nephrologists certified in Hemodialysis Vascular Access Procedures by the American Society of Diagnostic and Interventional Nephrology (ASDIN). To date, he has performed over 5,000 procedures and has helped train several other Interventional Nephrologists. Dr. Yu specializes in all aspects of hemodialysis vascular access including AV fistula maturation, chest port placement and varicose vein treatment.

Pradip Sahdev, M.D., F.A.C.S., R.P.V.I. has practiced General and Vascular Surgery in Waldorf, M.D. since 1995. He started Advanced Surgicare in 2003, which provides a variety of surgical and diagnostic services to the residents of Southern Maryland. His mission is to blend state-of-the-art medical technology and research with a deidication to patient welfare and healing to provide patients with the best possible surgical care. Dr. Sahdev joined V.A.C. in 2011. He is Board Certified in General Surgery, Enovascular Medicine, Vascular Interpretation, and Phlebology. He has been Chief of Surgery at Civista Hospital, and Chairman of Charles County Medical Society.

Geoffrey L. Risley, M.D., F.A.C.S.
VAC of Jacksonville

Dr. Risley is a board certified vascular surgeon with expertise in endovascular management of all aspects of arterial and venous disease. He regularly trains other physicians nationwide in new endovascular techniques and lectures frequently on both arterial and venous disease. He is actively involved in clinical trials studying the latest technical advances in endovascular surgery. He is a graduate of the University of Florida College of Medicine and completed his vascular surgical fellowship at the University of North Carolina at Chapel Hill. He studied venous disease with his first partner, Dr. John Bergan, a pioneer of modern vein care in San Diego. In 2003 he joined the physicians of Cardiothoracic & Vascular Surgical Associates and serves as the medical director of The Jacksonville Vein Center. Dr. Risley is a fellow of the American College of Surgeons, and a member of the Society for Vascular Surgery, the Southern Association for Vascular Surgery, the Florida Vascular Society, the Jacksonville Vascular Society, the American Venous Forum and the American College of Phlebology.

Sarah Clark, M.D.

Sarah Clark, M.D. joined Cardiothoracic & Vascular Surgical Associates in 2014. Dr. Clark is an expert in all aspects of vascular medicine, including surgical and endovascular treatment of all vascular diseases. She graduated from the University of Maryland School Of Medicine, completed her general surgery residency at the University of South Florida, followed by a vascular surgery fellowship at Penn State University. Erin Moore, M.D., F.A.C.S.
VAC of Jacksonville

Dr. Moore is an expert at minimally invasive vascular surgery who specializes in endovascular procedures at Baptist Medical Center. Dr. Moore graduated from the University of Texas Southwestern Medical School in Dallas and completed his residency in surgery at the Naval Medical Center in San Diego before undergoing a fellowship in vascular and endovascular surgery at the University of Kentucky Chandler Medical Center. Dr. Moore was appointed department head for vascular surgery at the Naval Medical Center Portsmouth, Virginia and is an assistant professor of surgery, Uniformed Services University of the Health Sciences.

Following his military service, he joined the Cardiothoracic & Vascular Surgical Associates out of a friendship with Dr. Risley, formed in San Diego years earlier. Dr. Moore directs vascular and endovascular surgery at Baptist Medical Center–Downtown. The American Board of Surgery, the American Board of Surgery–Vascular Surgery and the Board of Vascular Medicine, certifies him. Dr. Erin Moore is a fellow of the American College of Surgeons, a member of the Society for Vascular Surgery, the International Society of Vascular Surgery, the Jacksonville Vascular society and the Southern Association for Vascular Surgery.

Alfred Harding, M.D., F.A.C.S., R.P.V.I.

Dr. Harding received his medical degree from the University of Missouri–Columbia. He completed his residency in general surgery and fellowship in vascular surgery at the University of Missouri–Columbia where he also served on the faculty before moving to Jacksonville in 1990. He has been instrumental in developing the Health-Grade rated No. 1 vascular surgery program at St. Vincent’s Medical Center, and has served as the chief of the section of cardiothoracic and vascular surgery and the immediate past president of the medical staff. He has extensive experience in the repair of abdominal aortic aneursyms and endovascular peripheral vascular surgery. Dr. Harding has a special interest in non-invasive diagnosis of vascular disease and is a registered physician in vascular interpretation (R.P.V.I.). Dr. Harding is a fellow with the American College of Surgeons, a member of the Society for Vascular Surgery, Southern Association for Vascular Surgery, Society for Clinical Vascular Surgery, Jacksonville Vascular Society and the Florida Vascular Society.

Danny Vo, M.D.

Dr. Vo is a graduate of The Medical College of Wisconsin. He completed his residency at the University of Chicago and his fellowship in vascular and endovascular surgery at the Mayo Clinic in Rochester, Minnesota. Dr. Vo was on staff at the Mayo Clinic–Rochester in the department of vascular surgery until he accepted a position as Assistant Professor of Vascular Surgery at Shands–Jacksonville, a part of the University of Florida. In 2011, he joined the Cardiothoracic & Vascular Surgical Associates and maintains privileges at St. Vincent’s, Baptist, Memorial, Orange Park and St. Luke’s Hospitals. Dr. Vo is board certified by the American Board of Surgery and the American Board of Surgery–Vascular Surgery. Jorge A. Salazar, M.D.
VAC of Memphis and Bolivar

Dr. Salazar is a graduate of Rhodes College at Memphis. He received his medical training at the University of Tennessee in Memphis. His training included an Internship in Internal Medicine at Metro Health Medical Center in Cleveland Ohio. He completed his Residency in Diagnostic Radiology and a Fellowship in Cardiovascular & Interventional Radiology at the University of Tennessee at Memphis. Dr. Salazar is an Instructor in Diagnostic Radiology at the Health Science Center, University of Tennessee.

He is a member of the Radiological Society of North America, the American College of Radiology, the Memphis Vascular Society, the Memphis Roentgen Society, The American Medical Association, the Memphis & Shelby County Medical Society, the Tennessee Medical Association and the National Network of Latin American Medical Students. Dr. Salazar specializes in all aspect of hemodialysis access maintenance, peripheral arterial disease, arm and chest port placement, uterine fibroid embolization, varicose and spider vein treatment.

Salil Joshi, M.D.
VAC of Memphis and Bolivar

Dr. Salil Joshi first came to the Memphis area in 2002 for a Cardiovascular and Interventional Radiology Fellowship at the University of Tennessee, Memphis. Following that, he completed a fellowship in Cardiovascular and Interventional Pediatric Radiology in 2005. He has more than 10 years experience as a Board Certified Interventional Radiologist.

Before coming to Vascular Access Centers, he consulted at St. Jude’s Children’s Hospital, LeBonheur Children’s Hospital, and Methodist Hospital in Memphis, Tennessee. Dr. Joshi also served as program director for the UT Radiology Fellowship Program from 2009-2015 and started the Pediatric Interventional Oncology program at St. Jude’s Children’s Hospital. Dr. Joshi competed medical school in 1992 at D.Y. Patil Medical College in Kolhpur, India. He went on to finish a Radiology Residency at Lokmanya Tilak Medical College in Bombay, India. Dr. Joshi performs procedures at both the Midtown and East Memphis locations, as well as the Cleveland, Mississippi facility. He is married and has one son.

LOUISIANA

Matthew Sanger, M.D.
VAC of New Orleans

Dr. Sanger has been taking care of dialysis patients in New Orleans since 2004 and has been with Vascular Access Centers since 2008. Before this he completed a fellowship in nephrology at the prestigious Johns Hopkins University. Dr. Sanger completed his internal medicine training at Bellevue Hospital and the New York University Medical Center in Manhattan. Dr. Sanger graduated from the University Of Pittsburgh School Of Medicine in 1998 and obtained his undergraduate degree from the University of Pennsylvania.

The disruption to the health care system after hurricane Katrina posed many challenges for the dialysis patients of the New Orleans region. Dr. Sanger has been proud to be a part of the rebuilding of that system and has taken a special interest in insuring that dialysis patients have speedy access to unparalleled interventional vascular care. Dr. Sanger is a firm believer that the best patient outcomes occur when the advantages of technology are combined with humane and empathetic care.

Philip Gardner, M.D.
VAC of North Shore

Dr. Gardner is a Vascular Surgeon who graduated from Baylor College of Medicine in Houston. After a General Surgery Residency at the US Public Heath Service Hospital in New Orleans, he trained in vascular surgery at the Ochsner Clinic in New Orleans. He is Board Certified in Surgery, and a Fellow of the American College of Surgeons. Dr. Gardner is on the active Medical Staff of several local acute care hospitals. He has served on several medical executive committees and as Chief of Staff at St. Tammany Parish Hospital and Lakeview Regional Medical Center. He served as the Medical Staff Representative on the Hospital Board of St. Tammany Parish Hospital for seven years. Dr. Gardner obtained a Masters Degree in Medical Management from the Tulane University school of Public Health and Health Systems Management in 2008. He holds the Registered Vascular Technologist (RVT) certification in Vascular Ultrasound.

Dr. Gardner has been a leader in bringing the latest techniques in the treatment of vascular disease to his community. The first arterial balloon angioplasty, the first intra-vascular stent placement, and the first endovascular abdominal aortic aneurysm repair in the Covington community were performed by Dr. Gardner. His vascular surgical and angiographic experiences make him uniquely qualified to perform outpatient vascular access procedures. His vast experience in the surgical placement of dialysis shunts, fistulas, ports and catheters, makes him a uniquely qualified angiographer. His experience with the surgical treatment of peripheral arterial occlusive disease makes him qualified to evaluate all treatment options. He is the most experienced actively practicing vascular surgeon in his community.

Gregory D. Crenshaw, M.D.
VAC of NOLA and NSLA

Gregory D. Crenshaw, M.D. is a fellowship trained vascular surgeon. He specializes in both General Vascular and Endovascular Surgery. Dr. Crenshaw received his undergraduate degree in Biology from Gustavus Adolphus College in 1996. He received his Doctorate of Medicine from University of Mississippi in 2001. He went on to finish a Family Medicine Residency at the University of Mississippi in 2004.

He started his surgical career at Ochsner Clinic Foundation in New Orleans LA. He completed General Surgery training at Ochsner in 2012. He then did a Vascular Fellowship at Ochsner in 2012. While in residency he did research in treatment of esophageal cancer and in fellowship he participated in Carotid Arterial Research. He received the Alton Ochsner Leadership award in 2012 which goes to the resident/fellow who exemplifies both clinical and leadership excellence. He also received the President’s Award in 2012 from the Southern Association of Vascular Surgery for an outstanding clinical paper on the Economics of Carotid Endarterectomy.

Dr. Crenshaw’s interests include Peripheral Vascular interventions and the creation and maintenance of dialysis accesses. He is currently on staff at East Jefferson Hospital in Metairie Louisiana, New Orleans East Hospital, and Lakeview Regional Medical Center in Mandeville Louisiana.

Amiel Moshfegh, M.D.
VAC of Downey

Dr. Malwinder Singha graduated in 1993 from Northwestern University in Evanston, IL with a Bachelor's degree in Electrical Engineering and worked as an Electrical Engineer for two years before entering medical school. In 1999, he earned his medical degree from Rush University Medical School in Chicago, IL. He completed an internship in internal medicine with focus in transplant medicine at University of Pittsburgh before completing his residency in Diagnostic Radiology at St. Vincent Hospital in 2004. In 2005, he completed a fellowship in Vascular and Interventional Radiology at Rhode Island Hospital of Brown University in Providence, RI. He obtained additional training at Hospital of University of Pennsylvania and completed training in advanced cardiovascular imaging. Dr. Singha is board certified in Diagnostic Radiology as well as holds certification in Vascular and Interventional Radiology and is also certified as a registered physcian in vascular interpretation. Dr. Singha also holds ACC level II certification.

After fellowship, Dr. Singha has worked at both community hospitals and academic centers including most recently at Loma Linda University Medical Center - Murrieta. Dr. Singha is experienced in both vascular and non vascular treatment utilizing most recent advances in endovascular techniques.

Dr. Singha continues to be on the forefront of endovascular techniques and is a member of the Society of Interventional Radiology and North American Society of Cardiovascular Imaging. In his free time, Dr. Singha likes to play golf and travel.
